Figure 2 graphically shows the effects of misclassification by the comparator on the interpretation of diagnostic performance. This figure was generated from a simulation of 100 negative Truth ground samples and 100 positive Truth samples. Panel A shows actual test performance (0% comparative ranking), while Panel B shows the effect of random injection of 5% misclassification in comparison calls. The quantitative results of this specific simulation are compiled in Table 1. In order to assess the extent of the obvious differences in the declassification rates proposed in this table, we conducted a more in-depth review that varied the number of simulated samples (test size) (S5 Supporting Information, “Decrease in apparent in performance of index test, with 5% noise injected into comparator”). As expected, we found all confidence intervals by decreasing with the size of the tests. For example, a test result above a certain threshold could be considered a positive call, and a doctor`s opinion that a patient is disease-free could be considered a negative call. Tests with binary results are generally evaluated based on the sensitivity and specificity that are inherent in the test. The objective definition of sensitivity and specificity requires a standard of reference — a test generally recognized as the best method available to determine the existence or absence of a condition. If no benchmark is available, sensitivity and specificity are defined as a positive percentage agreement (AAE) and a negative percentage agreement (NPA) with another developer`s choice test. Sensitivity, specificity, prevalence of the disease, positive and negative forecast value and accuracy are expressed in percentages. A total of 100 negative Ground Truth patients and 100 positive Ground Truth patients were considered. In Panel A, there is no error in the classification of patients (i.e. the comparator perfectly corresponds to the truth of the soil). Panel B assumes that, at random, 5% of the comparator`s classifications deviate poorly from the truth of the ground. The difference in the distribution of test results (axis y) between the panels in this figure leads to a significant underestimation of diagnostic performance, as shown in Table 1. In medicine and epidemiology, the effects of classification uncertainty on apparent test performance are often referred to as “information bias,” “classification biases” or “undifferentiated bias” and are considered under other names in other areas [8-10].

These terms refer to the fact that, as classification uncertainty increases, there will be a growing gap between actual test performance and empirical measures of test performance, such as sensitivity, specificity, negative forecast value (NPV), positive predictive value (APP) or surface under the receiving operating line (ROCC).
